小孩染流感帶病毒較久傳播期可長達10天

（中央社記者陳偉婷台北22日電）嬰幼兒是流感重症高危險群，台大醫院小兒部感染科主治醫師呂俊毅說，兒童是流感易感族群，感染後較易發燒，研究也顯示小孩體內帶病毒時間比成人長，傳播期更可長達10天。

季節性流感是因感染人類流感病毒所引發的呼吸道疾病，多數國家每年均會發生週期性流行，目前季節性流感是指感染H1N1、H3N2的A型流感病毒或B型流感病毒。在台灣，流行期約從11月至隔年3月間，在農曆春節前後達到高峰。

台灣疫苗推動協會秘書長、台大醫院小兒部感染科主治醫師呂俊毅今天在記者會表示，每年全世界每5人就有1人會得到流感，因流感病毒具有高度變異性，很容易導致大流行，且傳染力極強，最新研究發現，流感患者不需咳嗽，僅單純呼吸就可讓流感病毒微粒懸浮於空氣中導致傳散。

呂俊毅也指出，小孩更是造成流感流行的關鍵，從健保資料統計，0至4歲小孩急性上呼吸道感染和流行性感冒就診率是全年齡之冠；小孩染流感後發燒比例也最高，研究發現，95%14歲以下小孩會因為流感發燒，但成人得到流感不見得發燒。

不僅如此，呂俊毅表示，小孩感染流感後，病毒停留在身上的時間最久，比成人約多出4天；根據衛生福利部疾病管制署資料，罹患流感者，在發病前一天到症狀出現後的3到7天都可能傳染給別人，但幼童的傳播期更長，可以長達7到10天。

根據疾管署季節性流感防治工作手冊，流感的可傳染期因人而異，研究發現，成年感染者在發病前24至48小時就開始排放病毒，但量較低，病毒排放的高峰是發病後 24至72小時，直至發病後第5天。但免疫不全者，排放病毒的期間可能達數週或數月；兒童也較早開始排放病毒，且量較多，時間較久，最長可達21天。

施打流感疫苗是防範流感最好的方式，政府今年提供的3價疫苗涵蓋H1N1密西根株（與2017年一致）、H3N2為新加坡株（取代2017年的香港株）、B型為維多利亞株系列的克羅拉多株（取代2017年的布里斯本株）。四價疫苗則多了B型山形株。

呂俊毅指出，有些民眾對流感有迷思，如認為流感季還沒來，可等快要流行了再打疫苗；但流感流行無法預期，打完疫苗需要2週才會產生保護力，還是應該儘早施打。

民眾可能較易輕忽B型流感，呂俊毅說，B流過去在台灣也有造成流行，且B型也有導致重症風險，民眾可考慮接種保護力更廣泛的4價流感疫苗。（編輯：黃于）1071022

李亦伸觀點：詹皇東冠之路，非常懷念小老弟厄文

克里夫蘭騎士104比100印第安那溜馬，「詹皇」詹姆斯(LeBron James)32分13籃板7助攻率領騎士將系列賽打成2比2平手，暫時保住東冠之路契機。

如果不是G3騎士在上半場領先溜馬17分，全場最多領先19分的優勢下被溜馬完成逆轉，系列賽目前應該是3比1，騎士聽牌。

詹皇不失常還能扛起騎士贏球，若失常沒有人能頂上攻擊

詹皇東冠之路，走得非常不順，溜馬很難纏，頑強又有韌性，這是事實，但騎士進攻打法太單調，除了「核武」詹皇，全隊進攻端缺少一個組織者和進攻發起點，也是騎士系列賽打得如此掙扎主因。

騎士關鍵球、決勝球、拉鋸戰全部都要仰賴詹皇打出狀況，只要詹皇失手，騎士就可能跌入困境，比賽難以挽回。

本季失去明星控衛「KI」厄文(Kyrie Irving)，騎士和詹皇遭遇的困難就在這裡。前三季詹皇可以自己打，場上休息時間或板凳時間，一場比賽至少有20分鐘可以換KI去主導比賽和負責進攻得分，騎士進攻和打法可以得到解決。一旦場上是詹皇+KI雙核心主導進攻，騎士戰力加倍難擋。

KI突破與單打能減輕詹姆斯場上壓力，是騎士東冠三連霸主因

KI無與倫比的個人能力、突破和單打技巧，就是騎士東冠三連霸的X因子，KI不需要太多戰術，不需要詹皇為他創造機會，他一個人就能完成得分和各種進攻手段。

現在這支騎士最大罩門就是少了KI，除了詹皇自己打，切傳組織帶動隊友，完全沒有另一門可靠武器和穩定進攻方式，一旦詹皇手氣不順，關鍵球打不進，隊友也沒有能力靠個人單打取分，騎士進攻就會被溜馬一一瓦解。

沒有KI，騎士戰力和韌性足足下滑一個檔次，這也是騎士打溜馬打得如此辛苦原因。

G4騎士就是贏在詹皇的全程爆發表現和克拉克森(Jordan Clarkson)+柯佛(Kyle Korver)單打、三分球命中。

美國TNT電視球評巴克利(Charles Barkley)認為：「如果溜馬在G4主場拿不下來，這個系列賽溜馬就會失去晉級最重要契機。」

溜馬還會給騎士製造很大威脅，騎士想要淘汰溜馬也沒那麼容易，溜馬還需要更多「運氣」，這些運氣包括詹皇的稍稍失準，或者騎士外圍射手沒有瘋狂爆發，溜馬就有機會在系列賽撂倒騎士。

詹皇東冠之路少了KI，情勢局面都在失控，不管是這一輪面對溜馬，還是下一輪可能面對暴龍或巫師，詹皇隨時可能跌跤，即使詹皇非常幸運連4打下東冠，騎士這套陣容和進攻，也很難跟西區冠軍進行至少6場對決。

當初快意恩仇送走厄文，季後賽愈打愈想念KI

溜馬系列賽給騎士不斷製造麻煩，相信所有騎士球迷心裡最想念的就是KI，如果騎士教練泰隆魯(Tyronn Lue)不能在出牌和奇兵牌上有不同組合，出牌更有效率，只靠詹皇一個人帶隊，最終可能會累死詹皇，同時搞砸騎士這麼好的天賦。
